Background:
Outpatient antimicrobial therapy (OPAT) is managed by a variety of teams, but primarily through an infectious disease clinic. At our medical center, OPAT monitoring is performed telephonically by pharmacists through a collaborative practice agreement under the supervision of an infectious disease physician. The effect of telephonic monitoring of OPAT by pharmacists on patient outcomes is unknown.
Methods:This retrospective cohort study was conducted between July 2017 and July 2018 at a 350-bed academic medical center and included adult patients discharged home on IV antibiotics or oral linezolid. The experimental group comprised patients discharged with a consultation for the OPAT management program, whereas the control group comprised patients discharged home without a consultation. The primary outcome was 30-day readmission.
Results:In total, 399 patients were included: 243 patients in the OPAT management program group and 156 patients in the control group. The 30-day readmission rates were similar in each cohort (20% vs 19%; P = .8193); however, the 30-day readmission rates were lower in the OPAT management program for patients discharged on vancomycin (19.4% vs 39.1%; P = .004).
Conclusions:We did not find a difference in 30-day readmissions between patients receiving pharmacy-driven OPAT management services and those who did not. Patients receiving vancomycin via OPAT had lower 30-day readmissions when included in the pharmacist-driven OPAT management program. Institutions with limited resources may consider reserving OPAT management services for patients receiving antimicrobials that require pharmacokinetic dosing and/or close monitoring.

Environmental uncertainty is at the core of much of human activity, ranging from daily decisions by individuals to long-term policy planning by governments. Yet, there is little quantitative evidence on the ability of non-expert individuals or populations to forecast climate-related events. Here we report on data from a 90-year old prediction game on a climate related event in Alaska: the Nenana Ice Classic (NIC). Participants in this contest guess to the nearest minute when the ice covering the Tanana River will break, signaling the start of spring. Previous research indicates a strong correlation between the ice breakup dates and regional weather conditions. We study betting decisions between 1955 and 2009. We find the betting distribution closely predicts the outcome of the contest. We also find a significant correlation between regional temperatures as well as past ice breakups and betting behavior, suggesting that participants incorporate both climate and historical information into their decision-making.

Objective:
To utilise a community-based participatory approach in the design and implementation of an intervention targeting diet-related health problems on Navajo Nation.
Design:A dual strategy approach of community needs/assets assessment and engagement of cross-sectorial partners in programme design with systematic cyclical feedback for programme modifications.
Setting:Navajo Nation, USA.
Participants:Navajo families with individuals meeting criteria for programme enrolment. Participant enrolment increased with iterative cycles.
Results:The Navajo Fruit and Vegetable Prescription (FVRx) Programme.
Conclusions:A broad, community-driven and culturally relevant programme design has resulted in a programme able to maintain core programmatic principles, while also allowing for flexible adaptation to changing needs.
Optimal Grazing Termination Date for Dual-Purpose Winter Wheat Production

Dual-purpose winter wheat (fall-winter forage plus grain) production is an important economic enterprise in the southern Great Plains. Grazing termination to enable grain production is a critical decision. The objective is to determine the optimal grazing termination date for dual-purpose wheat. The value of knowing the occurrence of first hollow stem (FHS), a wheat growth threshold for grazing termination, is also determined. Results indicate that for most price situations grazing should be terminated at or before FHS. Marginal wheat returns from extended grazing were negative and the value of FHS information ranges from $1.50 to $10 per acre.
